A 4x4x4 truncated like a cuboctahedron but deeper.
A 4x4x4 was truncated on its corners. This results in a shape which consists of 6 squares and 8 (non-regular) hexagons. To be precise the shape of this puzzle is not the cuboctahedron. The cutting depth is deeper than the archimedean solid. This was done to affect the face pieces of the 4x4x4 which can now be differentiated from each other.
